◆Introduction
１．This application form is for households with children born between April 1 and December 31, 2022.
２．To apply, you must answer a questionnaire, which takes about 10 to 15 minutes in total.

◆What to prepare before entering
・Image of the applicant's identification document (driver's license, passport, health insurance card, My Number card, residence card, etc.)
・Image of documents confirming the receiving account (in the name of the applicant only) (The part where we can check the bank name, branch name, account number, account holder name (kana) of the receiving account, such as a bankbook or cash card)　

◆Eligible persons and applicants
【During pregnancy】Those who meet all of the following requirements are eligible.
１．Mothers of children born between April 1st and December 31st, 2022 who reside in Toyohashi City as of the date of application
２．Those who had an address in Japan during pregnancy, and who submitted the notification of pregnancy in Japan.
※Individuals who have been overseas continuously from pregnancy to childbirth are not eligible for the benefits for reporting pregnancy.
【After birth】Those who meet the following requirements are eligible.
　Those who are raising children born between April 1 and December 31, 2022, and who reside in Toyohashi City as of the date of application
※Children who were born during the above period and subsequently died are also eligible for postnatal benefits.
